What do I need to submit if I had approved time off for the whole work week & did NOT work?

Regardless of how you typically document and submit your facility time, documentation entails a supplemental paper timecard indicating the shifts or days you had approved time off. This timecard does NOT need to be signed.

What do I need to submit if I had approved time off for part of the work week & did work?

  1. You will need to submit your facility time the same way you usually do, ensuring you meet the same requirements as before, AND

  2. You will need to submit a supplemental paper timecard indicating the shifts or days you had approved time off. This timecard does NOT need to be signed.
