How do I upload a completed reference?

How to upload a previously completed reference to your Trusted Profile.

Can I use a prior reference?

Do you have a recently completed reference? We can accept prior references that meet the following criteria:

  • The reference form was completed within the past 24 months

  • The reference pertains to a position you have held in the past 24 months

❗ Some facilities or hiring managers may have stricter timeframe requirements and therefore require references to be completed and from employment within the past 12 months.

  • The reference must ask and answer your eligibility for rehire. This means that if given the opportunity would your reference rehire you. Your reference form would need to ask and answer the question, "Would you rehire this candidate?"

  • The reference must be from a charge, manager, or supervisor. No peer or personal references are accepted.

How can I upload a completed reference to my profile?

❗ You must have your work history entered in first before trying to upload a previously completed reference. If your work history is blank there will be no options in the drop-down for you to select.

First things first, make sure you're on your me tab and navigate to your profile.

Under your profile locate the reference section.

Click Add a reference

You'll be prompted to select from one of two options, select upload reference

You will be prompted to fill out your reference's details, confirm you agree with the statement, upload the reference, and then click done to save the reference to your profile. If the button is grayed out, check to be sure all of the fields are filled in.

Once you add the reference, you can view it on your profile under the reference section.

❗ No matter if you opt to request a reference or upload a previously completed reference form, your Care Team will be reviewing your references when you apply to Matches. To prevent any delays in reviewing your applications, you must ensure your previously completed reference form meets all of the requirements listed above.
