How do I add a health document?

First things first, make sure you're on the Credentials page.

Scroll down to the Health Documents section.

Click on + Add Health Document, and a form will slide out from the right.

Fill out all fields you see here.

Inputting a completed date will allow us to notify you when to renew a health document, which is typically yearly.

Be sure to upload the necessary file attachment(s) to prevent any delay in your submission.

As always, be sure to Save!

To cancel any changes you've made, simply click the left arrow in the top bar. You will be prompted "You have unsaved changes. Are you sure you want to leave without saving?" Just click 'OK' and your changes will not save.
