❗ Per Trusted’s Timekeeping policy, you are required to provide an attachment that reflects your facility’s corresponding record of your time each week. Failure to do so may result in a delay in pay.
Requirements for Acceptable Facility Time Documentation
Electronic Timekeeping System
If your primary facility timekeeping requirement is electronic, the attachment must be a picture of your weekly report and include:
Your name (preferred, but not required)
Dates worked
The specific clock-in/out times for each shift
The hour types for each shift (regular hours, on call, call back, etc)
What do I submit if my facility timekeeping requirement is electronic?
Picture of your electronic timekeeping report/weekly summary.

If you do not have access to a weekly report that completely summarizes your time, you can provide screenshot(s) of your in/out times & dates worked + a supplemental paper timecard that provides full detail of your week’s time that matches the total hours in your weekly screenshot (supervisor signature optional if paired with an electronic timekeeping report that includes the aforementioned items)
Examples of electronic timekeeping documentation that shows ALL of Trusted's timekeeping policy requirements:
❗ If you do not have access to a weekly report meeting the above criteria:
Please reach out to your manager to let them know you need access. If your manager is not able to give you access, fill out a paper timecard and ask that your manager sign it each week. If your manager or supervisor says they are unwilling or unable to sign your timecard, please send your Care Team a message via Inbox ASAP for assistance.
If attaching a paper timecard in lieu of electronic timekeeping documentation, you must also continue tracking your time according to your facility’s electronic timekeeping requirements (badging in and out, etc)
Signed Paper Timecard
If your primary facility timekeeping requirement is a signed paper timecard, it must include the following:
Your name
Dates worked
The specific clock-in/out times for each shift
The hour types for each shift (regular hours, on call, call back, etc)
A supervisor's signature
📎 If your facility doesn't have a timecard that they require you to use, please use this blank Trusted paper timecard.
