How do I add a health document?

  1. First, make sure you're on the Credentials page.

  2. Scroll down to the Health Documents section.

  3. Click + Add Health Document. A form will slide out from the right.

  4. Complete all required fields.

  5. Enter the completed date so we can remind you when it’s time to renew—most health documents need to be updated annually.

  6. Upload the required file attachment(s) to avoid any delays in processing.

  7. When you're finished, don’t forget to click Save!

To cancel any changes you've made, simply click the left arrow in the top bar. You will be prompted "You have unsaved changes. Are you sure you want to leave without saving?" Just click 'OK' and your changes will not save.
